Tattoo Guidelines
The State of Illinois requires you to be 18 years of age to acquire a tattoo.
The State of Illinois requires that all minors MUST be accompanied by their Parent or Legal Guardian when entering the Tattoo/ Piercing establishment.
Tattoo Pricing & Consults
Five-O Tattoo has a shop minimum of $60.
Consults are free of charge.
We require a deposit on larger tattoos of $50-$100.
Tattoo ID Requirements
We require one of these forms of I.D.:
State-Issued Drivers License
State-Issued ID Card
Passport
Military ID
Resident Alien Card
We cannot accept expired, torn, broken, or illegible forms of ID.
